ABSTRACT

The COVID-19 pandemic and government measures have challenged family functioning (FF) and eating behaviors. In non-pandemic scenarios, FF has shown a protective role in diet quality. This study aimed to explore the role of FF in family members' food consumption during the first wave of the COVID-19 pandemic in Chile. We conducted a cross-sectional, non-probabilistic study. Chilean adults (n= 821) answered an online survey based on the Family Adaptability and Cohesion Evaluation Scales (FACES) IV and a scale of food intake perceptions (non-processed and processed food). The survey also included an open space for comments (n= 147). Most participants had a medium or high FF category. Participants with higher FF had a lower risk of decreasing the consumption of non-processed foods (RR= 0.58 for fruits and RR= 0.45 for legumes) and lower risk of increasing processed foods (RR= 0.37 for sugar-sweetened beverages, RR= 0.46 for snacks, and RR= 0.64 for chocolates) when compared to lower FF participants and controlling for per-capita income. Participants commented on changes in FF: some had more conflicts and others were happy to be together during the pandemic. Most participants indicated that they maintained or increased non-processed and processed food consumption compared to the pre-pandemic period. They perceived having the opportunity of eating healthier during the pandemic. However, the stress they suffered produced them to eat unhealthily. Therefore, FF had a protective role in diet quality during the pandemic. Policymakers should consider FF when promoting healthy diets, especially in health crises.


La pandemia por COVID-19 y las medidas gubernamentales han desafiado el funcionamiento familiar (FF) y los hábitos alimentarios. En escenarios no pandémicos, el FF ha demostrado un papel protector en la calidad de la dieta. Este estudio tuvo como objetivo explorar el papel del FF en el consumo de alimentos de los miembros de la familia durante la primera ola de la pandemia de COVID-19 en Chile. Realizamos un estudio transversal, no probabilístico. Adultos chilenos (n= 821) respondieron una encuesta online basada en la Escala de Evaluación de Cohesión y Adaptabilidad Familiar (FACES) IV y una escala de percepción de consumo de alimentos (alimentos no procesados y procesados). La encuesta también incluyó un espacio abierto para comentarios (n= 147). La mayoría de los participantes tenían una categoría de FF medio o FF alto. Los participantes con mayor FF tenían un menor riesgo de disminuir el consumo de alimentos no procesados (RR= 0,58 para frutas y RR= 0,45 para legumbres) y menor riesgo de aumentar los alimentos procesados (RR= 0,37 para bebidas azucaradas, RR= 0,46 para snacks, y RR= 0,64 para chocolates) en comparación con los participantes de FF más bajos y controlando por ingreso per cápita. Los participantes comentaron cambios en sus FF y teniendo más conflictos, aunque algunos estaban felices de estar juntos durante la pandemia. La mayoría de los participantes indicaron que mantuvieron o aumentaron los alimentos procesados y no procesados en comparación con el período pre-pandemia. Ellos percibieron tener la oportunidad de comer más sano. Sin embargo, el estrés que sufrían les producía comer menos saludable. La FF tuvo un papel protector en la calidad de la dieta durante la pandemia. Quienes formulen políticas públicas deben considerar la AF cuando promuevan dietas saludables, especialmente en crisis de salud.

ABSTRACT

Abstract The aim of the current research is to analyze the coexistence of modifiable risk behaviors for cardiovascular disease (CVD) in 12-to-17-year-old adolescents living in Brazil and their influence on overweight. National, cross-sectional, school-based epidemiological study focused on estimating the prevalence of cardiovascular risk factors and metabolic syndrome in 12 to 17 year old adolescents enrolled in public and private schools in Brazilian counties accounting for more than 100 thousand inhabitants. The grade of membership method was used to identify the coexistence of risk factors among adolescents. The analytical sample comprised 71,552 adolescents. According to the two herein generated profiles, adolescents classified under Profile 2 have shown behaviors such as smoking, alcohol consumption and diet rich in Ultra-processed food intake ≥ 80% of the percentage of total caloric value. In addition, adolescents presenting CVD risk profile have shown increased likelihood of being overweight. The study has found coexistence of risk factors for CVD in Brazilian adolescents, with emphasis on tobacco smoking and alcoholic beverage intake. In addition, it heads towards the analysis of the association between CVD risk factors and health outcomes, such as overweight.


Resumo O objetivo desta pesquisa é analisar a coexistência de comportamentos de risco modificáveis para doenças cardiovasculares (DCV) em adolescentes de 12 a 17 anos residentes no Brasil e sua influência no excesso de peso. Estudo epidemiológico nacional, transversal, de base escolar, com foco em estimar a prevalência de fatores de risco cardiovascular e síndrome metabólica em adolescentes de 12 a 17 anos matriculados em escolas públicas e privadas de municípios brasileiros que somam mais de 100 mil habitantes. O grade of membership foi utilizado para identificar a coexistência de fatores de risco entre os adolescentes. A amostra analítica foi composta por 71.552 adolescentes. De acordo com os dois perfis gerados, os adolescentes classificados no Perfil 2 mostraram comportamentos como fumar, consumo de álcool e dieta rica em alimentos ultraprocessados ≥ 80% da porcentagem do valor calórico total. Além disso, adolescentes com perfil de risco para DCV mostraram maior probabilidade de apresentar excesso de peso. O estudo encontrou coexistência de fatores de risco para DCV em adolescentes brasileiros, com destaque para tabagismo e consumo de bebidas alcoólicas. Além disso, demonstra associação entre fatores de risco para DCV e desfechos de saúde, como o excesso de peso.

ABSTRACT OBJECTIVE To assess the availability of different promotional strategies applied for UPF sales in Brazilian food retailers. METHODS Information available on food packaging was gathered from all packaged products sold in the five largest food retail chains in Brazil in 2017. UPF were identified using the NOVA food classification system. From this sample, data related to promotional characteristics, nutrition claims and health claims were collected and coded using the INFORMAS methodology. Additional claims referring to the Brazilian Dietary Guidelines were also collected. RESULTS This study evaluated the packaging of 2,238 UPF, of which 59.8% presented at least one promotional strategy. Almost one third denoted a simultaneous use of different promotional strategies in the same packaging. Nutrition claims were the most commonly found promotional strategy, followed by health claims and the use of characters. The food subgroups comprising the highest prevalence of promotional strategies on their labels were: noncaloric sweeteners (100.0%), breakfast cereals and granola bars (96.2%), juices, nectars and fruit-flavoured drinks (92.9%), other unsweetened beverages (92.9%), and other sweetened beverages (92.6%). CONCLUSIONS Considering the poor nutritional quality of UPF, the widespread presence of promotional features on their packaging highlights the need for marketing restrictions on this kind of product.

@#Introduction: Cancer is the second leading cause of mortality globally. Processed and ultra-processed foods are reported to be high in energy, sugar, and unhealthy fats, while low in fibre, vitamins and minerals, that can increase the risk of cancer. Methods: A cross-sectional study involving adults aged 18-59 years was conducted in Peninsular Malaysia (n=122). Sociodemographic data were obtained through a questionnaire and lifestyle habits score for cancer prevention was assessed using MyGenomSihat©. The food frequency questionnaire used had four groups (i.e., unprocessed or minimally processed foods, processed culinary ingredients, processed foods, and ultra-processed foods) containing 53 food items to evaluate the frequency of food consumption. All data obtained were self-reported by the participants. Results: Mean age was 27.6±8.1 years with a majority (76.2%) having household income of less than <RM4,850. The results from MyGenomSihat© demonstrated that majority (53.3%) of subjects had average scores between 41-85, which is considered to be satisfactory lifestyle habits. Most Malaysian adults ate ultra-processed foods (32.9%) compared to processed foods (4.9%). However, there were no significant correlations between consumption of both processed and ultraprocessed food groups and lifestyle habits score for cancer prevention (r=-0.072, p=0.089 and r=-0.008, p=0.992, respectively). Conclusion: Most subjects had satisfactory lifestyle habits scores for cancer prevention. Interventions are needed to improve lifestyle habits among the targeted population for cancer prevention.

Resumen Introducción: el alarmante incremento de la obesidad en todo el mundo y en Costa Rica responde principalmente a modificaciones en la composición de la dieta habitual de las personas. La presente investigación tuvo como objetivo implementar un protocolo de alimentación formulado a partir de alimentos altamente procesados y de alta palatabilidad (APAP) consumidos por la población costarricense para, luego, evaluar sus efectos en la conducta alimentaria y en parámetros biométricos y bioquímicos. Metodología: ratas adultas macho Wistar se asignaron a dos grupos. Al primero se le administró alimento estándar para roedores (grupo DC) y al segundo, alimentos APAP (grupo APAP), durante ocho semanas. Resultados: a lo largo de ese periodo, los animales expuestos a los APAP exhibieron mayor ingesta y energía, caracterizadas por un alto consumo de grasas y uno menor de proteínas y fibra; además, mostraron un incremento significativo en los diversos parámetros de obesidad (e. g., peso corporal y ganancia de peso, índice de Lee y adiposidad central) y niveles descriptivamente superiores de glucosa y triglicéridos en sangre, pero notablemente menores de colesterol total. Conclusiones: los resultados indican que una dieta basada en los alimentos APAP más frecuentes en la población costarricense es capaz de inducir hiperfagia y obesidad. Así, este modelo constituye una herramienta prometedora para ahondar en el estudio de las factores neurobiológicos y metabólicos relacionados con la obesidad por el sobreconsumo de alimentos APAP.


Abstract Introduction. The alarming increase in obesity both worldwide and in Costa Rica is mainly due to changes in the composition of the usual diet of the population. The goal of our research was to implement a feeding protocol formulated from ultra-processed and highly palatable foods (UPHP) consumed by the Costa Rican population and to evaluate the effects of the UPHP diet on eating behavior and biometric and biochemical parameters. Methods: Adult male Wistar rats were assigned to two groups. One group was given standard rodent chow (DC group) while the other group received UPHP foods (UPHP group) for eight weeks. Results: Throughout this period, animals exposed to the UPHP diet exhibited higher food and energy intake characterized by high consumption of fat and lower consumption of protein and fiber. Animals in the UPHP group also showed a significant increase in obesity parameters (e.g., body weight and bodyweight gain, Lees index, and central adiposity). Furthermore, the UPHP group had descriptively higher levels of blood glucose and triglycerides and significantly lower levels of total cholesterol. Conclusions: Our results indicate that a feeding protocol based on the most frequent food choices of the Costa Rican population is capable of inducing hyperphagia and obesity. This model constitutes a promising tool to delve into the study of the neurobiological and metabolic factors related to obesity induced by overconsumption of UPHP foods.

Abstract Objectives: to verify the correlation between the consumption of ultra-processed food among mothers and children under two years of age and the main characteristics related to this consumption. Methods: cross-sectional study conducted in public health services. Three 24-hour recalls were applied to assess food intake. The ultra-processed food was grouped into: sugary drinks; meat; sauces and creams; dairy products; snacks; pastas; and mucilage. Themother's body mass index and waist/hip ratio, and the child's weight/height, height/age, weight/age and body mass index/age were calculated. The children's ultra-process frequency as correlated with: anthropometric dyadic variables; ultra-process frequency on breastfeeding. The children's average ultra-process intake was compared to pacifier, bottle, breastfeeding and socioeconomic status. Linear regression models were conducted. Results: 172 pairs were evaluated. Similarity was found in the mothers and children's consumption of ultra-processed products. The higher frequency of ultra-processed products was correlated with older child and the higher body mass/age index and weight/age index. Of the 39 ultra-processed food present in the mothers' diet, 22 were correlated to child's Conclusion: the consumption of ultra-processed food by children is similar to their mothers and correlates with higher z-score values of weight/age and body mass/age index.


Resumo Objetivos: verificar a correlação do consumo de alimentos ultraprocessados de mães e filhos menores de dois anos de idade e as principais características relacionadas a este consumo. Métodos: estudo transversal conduzido nos serviços públicos de saúde. Aplicaram-se três recordatórios de 24 horas para avaliar o consumo alimentar. Os alimentos ultraprocessados foram agrupados em: bebidas açucaradas; carnes; molhos e cremes; lácteos; lanches; massas; e mucilagens. Calculou-se o Índice de Massa Corporal e a relação cintura/quadril da mãe, índice peso/estatura, estatura/idade, peso/idade e índice de massa corporal/idade da criança. Correlacionou-se a frequência de ultraprocessados na alimentação das crianças com: variáveis antropométricas da díade; frequência de ultraprocessados na alimentação materna. Comparou-se a média do consumo de ultraprocessados das crianças com uso de chupeta, mamadeira, aleitamento materno e condição socioeconômica. Modelos de regressão linear foram conduzidos. Resultados: avaliou-se 172 pares. Foi verificada semelhança no consumo de ultraprocessados de mães e filhos. A maior frequência de ultraprocessados correlacionou-se a maior idade da criança e ao maior índice de massa corporal/idade e índice peso/idade. Dos 39 alimentos ultraprocessados presentes na alimentação das mães, 22 correlacionaram com os da criança. Conclusão: o consumo de ultraprocessados pelas crianças se assemelha ao das mães e correlacionase com maiores valores dos índices peso/idade e índice de massa corporal/idade.

ABSTRACT Objective To assess the association between the maternal diet, according to the degree of processing of food consumption, and birth weight for gestational age and sex. Methods A cross-sectional study with 300 women was conducted from February 2009 to 2011 from a maternity ward in Mesquita, Rio de Janeiro. The outcome was based on sex-specific birth weight for gestational age: small, adequate, or large. A validated food frequency questionnaire was used to estimate the food consumption during the 2nd and 3rd trimesters of pregnancy. The food intake was classified into three groups according to the degree of processing: 1) unprocessed or minimally processed foods and culinary ingredients (oil, fats, salt, and sugar), 2) processed foods, and 3) ultra-processed foods. Descriptive analyses were made to assess the tertiles of the percentage of energy intake of each food group on the outcome and on maternal and infant characteristics. Multinomial logistic regressions were used to test the association of the tertiles of food according to the degree of processing on the outcome (adequate, small, or large birth weight for gestational age and sex). Results The analysis of the food frequency questionnaire from the 300 women indicated that the mean percentage of kcal consumed from unprocessed and minimally processed food and culinary ingredients was 54.0%, while the percentages of energy from processed foods and ultra-processed foods were 2.0% and 44.0%, respectively. The highest tertile of consumption of unprocessed and minimally processed food and culinary ingredients had a protective effect on the prevalence of newborn large for gestational weight in relation to the lowest (OR: 0.13; 95% IC: 0.02 to 0.89; p=0.04). Conclusion High consumption of unprocessed and minimally processed food and culinary ingredients during the last six months of pregnancy might be a protective factor against having a newborn large for gestational weight when compared to mothers with the lowest consumption.


RESUMO Objetivo Avaliar a associação da dieta materna de acordo com o grau de processamento dos alimentos e o peso ao nascer segundo a idade gestacional e sexo. Métodos Estudo transversal com 300 mulheres captadas entre os meses de fevereiro de 2009 e 2011. Utilizou-se a classificação do peso ao nascer segundo sexo e idade gestacional para caracterizar os desfechos: pequeno, adequado ou grande. O questionário de frequência alimentar estimou o consumo durante o 2º e 3º trimestres da gestação. Os alimentos foram classificados segundo o grau de processamento: 1) alimentos in natura ou minimamente processados e ingredientes culinários (óleos, gordura, sal e açúcar), 2) alimentos processados e 3) alimentos ultraprocessados. Os tercis de energia das categorias descritas acima foram distribuídos segundo o desfecho e as características maternas e do recém-nascido. Adotou-se a regressão logística multinomial para analisar a associação do consumo de alimentos segundo o grau de processamento sobre os desfechos do peso ao nascer segundo a idade gestacional e o sexo (pequeno, adequado ou grande). Resultados A análise do questionário de frequência do consumo alimentar das 300 mulheres indicou que a contribuição de alimentos in natura ou minimamente processados e ingredientes culinários foi de 54,0%, enquanto que os percentuais dos grupos de alimentos processados e ultraprocessados foram 2,0% e 44,0%, respectivamente. O maior tercil de alimentos in natura ou minimamente processados e ingredientes culinários obteve efeito protetor para a prevalência de recém-nascidos grandes para a idade gestacional e o sexo em relação ao menor tercil (OR: 0,13; IC 95%: 0,02;0,89; p=0,04). Conclusão O maior consumo de alimentos in natura ou minimamente processados e ingredientes culinários durante a gestação pode ser um fator de proteção contra a ocorrência de recém-nascidos grandes para a idade gestacional e o sexo quando comparado com mães classificadas no menor tercil de consumo.

ABSTRACT

Abstract Objective: To describe the participation of the environment in the childhood obesity epidemic, since childhood obesity currently represents a great challenge, with high prevalence worldwide, including in Brazil. Data source: Survey of articles published in the last 10 years in PubMed, evaluating the interface between the environment and childhood obesity. Data synthesis: Recent studies show that the environment is very important in the etiopathogenesis of obesity and its comorbidities. Therefore, factors such as air pollution, exposure to chemical substances that interfere with the metabolism, excessive consumption of ultra-processed foods, changes in the intestinal microbiota, and sedentary lifestyle are associated with increased obesity, insulin resistance, type 2 diabetes, and changes in lipid metabolism. These factors have a greater impact on some stages of life, such as the first thousand days, as they affect the expression of genes that control the adipogenesis, energy expenditure, and the mechanisms for hunger/satiety control. Conclusions: Environmental aspects must be taken into account in the prevention and treatment of childhood obesity, both from the individual and the population point of view, with adequate and comprehensive public health policies.

ABSTRACT

ABSTRACT OBJECTIVE To study the association between ultra-processed food consumption and carbon and water footprints of the Brazilian diet. METHODS Cross-sectional analysis on data collected in 2008-2009 on a probabilistic sample of the Brazilian population aged ≥ 10 years (n = 32,886). Individual food intake was assessed using two 24-hour food records, on non-consecutive days. The environmental impact of individual diets was calculated by multiplying the amount of each food by coefficients that quantify the atmospheric emissions of greenhouse gases in grams of carbon dioxide equivalent (carbon footprint) and freshwater use in liters (water footprint), both per gram or milliliter of food. The two coefficients consider the food life cycle 'from farm to fork.' Crude and adjusted linear regression models and tests for linear trends assessed the association between the ultra-processed food contribution to total energy intake (quintiles) and the diet carbon and water footprints. Potential confounders included age, sex, education, income, and region. Total energy intake was assessed as a potential mediation variable. RESULTS In the crude models, the dietary contribution of ultra-processed foods was linearly associated with the carbon and water footprints of the Brazilian diet. After adjustment for potential confounders, the association remained significant only regarding the diet water footprint, which increased by 10.1% between the lowest and highest quintile of the contribution of ultra-processed foods. Additional adjustment for total energy intake eliminated this association indicating that the dietary contribution of ultra-processed foods increases the diet water footprint by increasing energy intake. CONCLUSIONS The negative impact of ultra-processed foods on the diet water footprint, shown for the first time in this study, adds to the negative impacts of these foods, already demonstrated regarding dietary nutrient profiles and the risk for several chronic non-communicable diseases. This reinforces the recommendation to avoid ultra-processed foods made in the official Brazilian Dietary Guidelines and increasingly in dietary guidelines of other countries.

ABSTRACT OBJECTIVE To describe the Nova score for the consumption of ultra-processed foods (UPF) and evaluate its potential in reflecting the dietary share of UPF in Brazil. METHODS This study was conducted in São Paulo with a convenience sample of 300 adults. Using a tablet, participants answered a 3-minute electronic self-report questionnaire on the consumption of 23 subgroups of UPF commonly consumed in Brazil, regarding the day prior the survey. Each participant score corresponded to the number of subgroups reported. The dietary share of UPF on the day prior to the survey, expressed as a percentage of total energy intake, was calculated based on data collected on a 30-minute complete 24-hour dietary recall administered by trained nutritionists. The association between the score and the dietary share of UPF was evaluated using linear regression models. The Pabak index was used to assess the agreement in participants' classification according to the fifths of Nova score and the fifths of dietary share of UPF. RESULTS The average dietary share of UPF increased linearly and significantly with the increase of the Nova score for the consumption of ultra-processed foods. We found a substantial agreement in participants' classification according to the fifths of the distribution of scores and the fifths of the dietary share of UPF (Pabak index = 0.67). Age was inversely associated with a relatively high frequency of UPF consumption (upper fifth of the distribution) for both score and dietary share of UPF. CONCLUSION The Nova score for the consumption of ultra-processed foods, obtained in a quick and practical manner, shows a good potential in reflecting the dietary share of UPF in Brazil.


RESUMO OBJETIVO Descrever o escore Nova de consumo de alimentos ultraprocessados e avaliar seu potencial para refletir, no contexto brasileiro, a participação desses alimentos na dieta. MÉTODOS Estudo realizado na cidade de São Paulo com amostra de conveniência de 300 adultos, que responderam, em cerca de três minutos, em um tablet, a um questionário eletrônico de autorrelato sobre o consumo, no dia anterior, de 23 subgrupos de alimentos ultraprocessados comumente consumidos no Brasil. O escore de cada participante correspondeu ao número de subgrupos reportados. A participação de alimentos ultraprocessados no consumo alimentar do mesmo dia, expressa como percentual da ingestão total de energia, foi calculada por meio das respostas dos participantes a recordatório alimentar completo de 24 horas aplicado em cerca de 30 minutos por nutricionistas treinados. A associação entre o escore e a participação de ultraprocessados na dieta foi estudada por modelos de regressão linear. A concordância na classificação dos participantes segundo quintos do escore e quintos da participação de alimentos ultraprocessados na dieta foi avaliada pelo índice Pabak. RESULTADOS O percentual médio de participação de alimentos ultraprocessados na dieta aumentou linear e significativamente com o aumento do escore Nova de consumo de alimentos ultraprocessados. Observou-se concordância substancial na classificação dos participantes segundo quintos da distribuição do escore e quintos da distribuição do percentual de participação de alimentos ultraprocessados na dieta (índice Pabak = 0,67). Relação inversa da idade com a frequência de consumo relativamente elevado de alimentos ultraprocessados (quinto superior da distribuição) foi observada tanto para o escore quanto para a participação de alimentos ultraprocessados na dieta. CONCLUSÃO O escore Nova de consumo de alimentos ultraprocessados, obtido de forma rápida e prática, apresenta bom potencial para refletir, no contexto brasileiro, a participação desses alimentos na dieta.

ABSTRACT

ABSTRACT OBJECTIVE To describe the magnitude of consumption of ultra-processed foods in the adult population (≥ 18 years old) in the capitals of the 27 federative units of Brazil, as well as its association with sociodemographic variables. METHODS Data used in this study stem from participants (n = 52,443) of the 2019 wave of the annual survey of the "National surveillance system for risk and protective factors for chronic diseases by telephone survey" (Vigitel). The consumption of ultra-processed foods was described based on a score, corresponding to the sum of positive responses to questions about consumption on the previous day of thirteen subgroups of ultra-processed foods frequently consumed in Brazil. Poisson regression models were used to describe the crude and adjusted associations between high consumption of ultra-processed foods (scores ≥ 5) and sex, age group, and level of education. RESULTS The frequency of high consumption of ultra-processed foods was 18.2% (95% CI 17.4-19.0). With or without adjustment for other sociodemographic variables, this frequency was significantly lower in females and decreased linearly with age. In the crude analysis, there was an increase in the frequency of high consumption from the lower level to the intermediate level of education and a decrease in this consumption from the intermediate level to the upper level. In the analysis adjusted for sex and age, the frequency of high consumption of ultra-processed foods was significantly lower at the higher level of education (12 or more years of study), with no differences between the other levels. CONCLUSION Ultra-processed foods are consumed with high frequency in the adult Brazilian population in the 27 capitals of the federation. Being male, younger and having less education than university are conditions that increase, independently, the consumption of these foods.


RESUMO OBJETIVO Descrever a magnitude do consumo de alimentos ultraprocessados na população adulta (≥ 18 anos) das capitais das 27 unidades federativas do Brasil e sua associação com variáveis sociodemográficas. MÉTODOS Os dados utilizados neste estudo provêm dos participantes (n = 52.443) da onda 2019 do inquérito anual do "Sistema nacional de vigilância de fatores de risco e proteção para doenças crônicas por inquérito telefônico" (Vigitel). O consumo de alimentos ultraprocessados foi descrito com base em escore correspondente à somatória de respostas positivas para questões sobre o consumo no dia anterior de treze subgrupos de alimentos ultraprocessados frequentemente consumidos no Brasil. Modelos de regressão de Poisson foram utilizados para descrever as associações bruta e ajustada do alto consumo de alimentos ultraprocessados (escores ≥ 5) com sexo, faixa etária e nível de escolaridade. RESULTADOS A frequência de alto consumo de alimentos ultraprocessados foi 18,2% (IC95% 17,4-19,0). Com ou sem o ajuste para as demais variáveis sociodemográficas, essa frequência foi significativamente menor no sexo feminino e diminuiu linearmente com a idade. Na análise bruta, evidenciou-se aumento na frequência de alto consumo do nível inferior para o nível intermediário de escolaridade e diminuição desse consumo do nível intermediário para o superior. Na análise ajustada por sexo e idade, a frequência de alto consumo de alimentos ultraprocessados foi significativamente menor no nível superior de escolaridade (12 ou mais anos de estudo), não havendo diferenças entre os demais níveis. CONCLUSÃO Alimentos ultraprocessados são consumidos com alta frequência na população brasileira adulta das 27 capitais da federação. Pertencer ao sexo masculino, ser mais jovem e ter escolaridade inferior à universitária são condições que aumentam, de forma independente, o consumo desses alimentos.

ABSTRACT The NOVA classification categorizes foods according to the extent of industrial processing. NOVA has been used in dietary guidelines of some countries including Brazil and Uruguay. This article aimed to investigate knowledge and perceptions of a sample of Brazilian adults regarding NOVA. A qualitative study was conducted in Dourados city, Mato Grosso do Sul, Brazil. First, participants (N= 24) were asked to classify a series of 24 pictures of foods and beverages using NOVA, which define the four major food groups: unprocessed or minimally processed foods; processed culinary ingredients; processed foods; and ultra-processed foods. Next, participants were asked to explain their classification through semi-structured interviews. Data from the classification activity were analyzed using non-metric multidimensional scaling and interviews using exploratory content analysis and summative content analysis. Participants seemed to understand NOVA in terms of food processing, food production, and additives used. They easily identified unprocessed or minimally processed foods and ultra-processed foods; processed culinary ingredients and processed foods were harder to identify. Professionals, researchers and government organisations in Brazil or abroad could consider the results of this study in order to optimize this tool's potential for research and policy in nutrition and public health.


RESUMEN La clasificación NOVA ordena los alimentos según su grado de procesamiento industrial. NOVA ha sido utilizado en las guías alimentarias de algunos países incluyendo Brasil. Investigamos el conocimiento y las percepciones de un muestreo de adultos brasileños con respecto a NOVA. Se realizó una investigación cualitativa en la ciudad de Dourados, Mato Grosso do Sul, Brasil. Los participantes (N= 24) clasificaran 24 imágenes de alimentos y bebidas en los cuatro grupos de NOVA: alimentos sin procesar o mínimamente procesados; ingredientes culinarios procesados; alimentos procesados; y alimentos ultraprocesados. Luego, se les pidió que explicaran su clasificación a través de entrevistas semiestructuradas. Los datos de esa actividad se analizaron mediante escala multidimensional no métrica y las entrevistas mediante análisis de contenido exploratorio y análisis de contenido sumativo. Los participantes entienden la NOVA en términos de procesamiento y producción de alimentos, y uso de aditivos. Ellos identificaron fácilmente los alimentos sin procesar o mínimamente procesados y los alimentos ultraprocessados pero no los alimentos de los otros grupos. Investigadores y organizaciones gubernamentales en Brasil y en el extranjero podrían tener en cuenta estos resultados para optimizar el potencial de esta herramienta para la investigación y las políticas en nutrición y salud pública.

RESUMEN La alimentación es un factor de riesgo modificable para el desarrollo de cáncer. El consumo de alimentos procesados y ultraprocesados se ha asociado a mayor riesgo de enfermedades crónicas no transmisibles. El objetivo de esta revisión es evaluar sistemáticamente la evidencia publicada sobre la relación entre el consumo de alimentos procesados y ultraprocesados con el desarrollo de distintos tipos de cáncer. Se realizó una búsqueda sistemática en la base de datos de Pubmed en los últimos 10 años. Se incluyeron estudios de cohorte, caso control o transversales que evaluaran riesgo de cáncer, no recidiva, en adultos y midieron la alimentación a través de encuestas o patrones dietarios. Dos autores revisaron de forma independiente títulos y resúmenes para seleccionar los artículos, posteriormente ambos realizaron una revisión de texto completo, extrajeron los datos y analizaron la calidad metodológica de los estudios incluidos. Los resultados contemplan las conclusiones de 18 estudios, cuatro estudios de cohorte, 13 caso-control y uno transversal. Catorce de los estudios incluidos asocian positivamente la ingesta de alimentos procesados y ultraprocesados con el desarrollo de algunos tipos de cáncer.


ABSTRACT Diet is a modifiable risk factor for the development of cancer. Consumption of processed and ultra-processed foods has been associated with an increased risk of chronic non-communicable diseases. The objective of this review is to systematically evaluate the published evidence on the relationship between the consumption of processed and ultra-processed foods with the development of different types of cancer. A systematic review of the Pubmed database was carried out for articles published in the last 10 years. Cohort, case-control or cross-sectional studies that evaluated cancer risk in adults, no cancer recurrence, and studies that evaluated diet through surveys or dietary patterns were included. Two authors independently reviewed titles and abstracts to select articles, and subsequently carried out a text review. Data was extracted and the methodological quality was analyzed of the included studies. The results contemplate the conclusions of 18 studies, four cohort studies, 13 case-control and one cross-sectional. Fourteen of the included studies showed a positive association between the intake of processed and ultra-processed foods and the development of cancer.

ABSTRACT

Nonalcoholic fatty liver disease (NAFLD) has become one of the most common chronic liver disease in the world. The environment and the susceptibility of the host genetic background determine the disease phenotype and affect its progress. Dietary factors play an extremely important role in the occurrence and development of metabolic diseases. The food carries energy and information, which directly or indirectly affects the metabolism and function of the human body. Currently animal models of diet-related NAFLD have been very mature, but it is not perfect. Therefore, based on the complexity of NAFLD, in order to elucidate the pathogenesis of disease, look for potential therapeutic targets, evaluate the treatment, it is necessary to develop effectively and safely preclinical models in vitro and in vivo. We should focus on finding pathogenic animal models that are more consistent with human dietary patterns, which is important for further understanding of NAFLD disease. Therefor, this article reviews the current research progress of animal models of diet-related NAFLD, and discuss the importance of establishing new diet-related NAFLD models.

@#Introduction: There is an emergent increase of ultra-processed food consumption in developing countries including Malaysia which carries a likelihood towards the increase of obesity. However, few studies have been done in relating between ultra-processed consumption and obesity, especially in Malaysia. Therefore, this study aims to determine the consumption of ultra-processed foods and its relationship with BMI and body fat percentage among university committee in Kuala Nerus, Terengganu. Methods: A cross-sectional study was performed among 167 individuals aged 18 to 59 years old in Kuala Nerus. Data were collected using a researcher-administered questionnaire which consisted of three different sections: socio-demographic profile, two days 24-hour dietary recall, and nutritional status assessment on BMI and body fat percentage. Consumption of ultra-processed foods were determined by classifying two days 24-hour dietary recall based on ultra-processed food classification. Data were analyzed using SPSS version 21.0 with p<0.05 to indicate significant. Results: The results showed Group 4 (ultra-processed foods) contributes 23% of overall daily energy consumption while Group 1+2 (unprocessed or minimally processed food + culinary ingredients) and Group 3 (processed foods) had contributed 75% and 2% respectively. Results showed that there was no relationship found between the ultra-processed food consumption and nutritional status through spearmen correlation at p<0.05. Conclusion: It is evidently clear from the findings that the consumption of ultra-processed foods is providentially still low among the respondents.

Abstract Objective: To analyze the prevalence of ultra-processed food intake among children under one year of age and to identify associated factors. Methods: A cross-sectional design was employed. We interviewed 198 mothers of children aged between 6 and 12 months in primary healthcare units located in a city of the metropolitan region of São Paulo, Brazil. Specific foods consumed in the previous 24 h of the interview were considered to evaluate the consumption of ultra-processed foods. Variables related to mothers' and children's characteristics as well as primary healthcare units were grouped into three blocks of increasingly proximal influence on the outcome. A Poisson regression analysis was performed following a statistical hierarchical modeling to determine factors associated with ultra-processed food intake. Results: The prevalence of ultra-processed food intake was 43.1%. Infants that were not being breastfed had a higher prevalence of ultra-processed food intake but no statistical significance was found. Lower maternal education (prevalence ratio 1.55 [1.08-2.24]) and the child's first appointment at the primary healthcare unit having happened after the first week of life (prevalence ratio 1.51 [1.01-2.27]) were factors associated with the consumption of ultra-processed foods. Conclusions: High consumption of ultra-processed foods among children under 1 year of age was found. Both maternal socioeconomic status and time until the child's first appointment at the primary healthcare unit were associated with the prevalence of ultra-processed food intake.


Resumo: Objetivo: Analisar a prevalência do consumo de alimentos ultraprocessados entre crianças com menos de um ano e identificar os fatores associados. Métodos: Foi realizado um estudo transversal. Entrevistamos 198 mães de crianças com idades entre 6 e 12 meses em unidades de atenção primária à saúde localizadas em Embu das Artes, uma cidade da região metropolitana de São Paulo, Brasil. Alimentos específicos consumidos nas 24 horas anteriores à entrevista foram considerados para avaliar o consumo de alimentos ultraprocessados. As variáveis relacionadas às características das mães e crianças e as unidades de atenção primária à saúde foram agrupadas em três blocos de influência cada vez mais proximal com o resultado. Foi realizada uma análise de regressão de Poisson de acordo com um modelo estatístico hierárquico para determinar os fatores associados ao consumo de alimentos ultraprocessados. Resultados: A prevalência de consumo de alimentos ultraprocessados foi 43,1%. As crianças que não eram amamentadas apresentaram maior prevalência de consumo de alimentos ultraprocessados, porém não foi encontrada diferença estatística. Menor nível de escolaridade materna (RP 1,55 [1,08-2,24]) e o fato de a primeira consulta da criança na unidade de atenção primária à saúde acontecer na primeira semana de vida (RP 1,51 [1,01-2,27]) foram fatores associados ao consumo de alimentos ultraprocessados. Conclusões: Foi encontrado consumo elevado de alimentos ultraprocessados entre crianças com menos de um ano. A situação socioeconômica materna e o tempo da primeira consulta da criança na unidade de atenção primária à saúde foram associados à prevalência de consumo de alimentos ultraprocessados.

ABSTRACT

Abstract: Objective: To evaluate the consumption of ultra-processed food and drink products and its association with the nutrient profile of the Colombian diet in 2005. Materials and methods: Food consumption based on 24-hour dietary records from 38 643 men and women was classified into four NOVA groups according to the extent and purpose of food processing. Results: Ultra-processed food and drink products represented 15.9% of the total energy daily intake, compared to 63.3% from minimally processed food, 15.8% from processed culinary ingredients, and 4.9% from processed food. Non-ultra-processed food items had a healthier nutritional profile compared to ultra-processed items in terms of contribution to total calories from protein, carbohydrates, total fat, saturated fat, free sugar, fiber and energy density. Conclusions: Ultra-processed food products have a less healthy nutrient profile than non-ultra-processed ones. An increase in the consumption of these foods must be prevented within Colombia.


Resumen: Objetivo: Evaluar el consumo de alimentos y bebidas ultraprocesadas y su asociación con el perfil nutricional dentro de la Encuesta Nacional de Nutrición de 2005 en Colombia. Material y métodos: El consumo de alimentos basados en registros alimentarios de 24 horas de 38 643 individuos se clasificó en cuatro grupos de NOVA. Resultados: Los productos de alimentos y bebidas ultraprocesados representaron 15.9% de la ingesta diaria total de energía, en comparación con 63.3% de los alimentos mínimamente procesados, 15.8% de los ingredientes culinarios procesados y 4.9% de los procesados. Los alimentos mínimamente procesados tenían un perfil nutricional más saludable en comparación con los artículos ultraprocesados en términos de contribución a las calorías totales, de proteínas, carbohidratos, grasa total, grasa saturada, azúcar libre, densidad de fibra y densidad de energía. Conclusiones: Los alimentos ultraprocesados tienen un perfil nutricional menos saludable que los alimentos no procesados.

ABSTRACT

This study provides basic data on how stress impacts the processed convenience foods purchase attitudes and the selection attributes of housewives. The stress consists of 3 factors, which were housework stress, family relation stress and economic stress. The processed convenience food purchase attitude consisted of 2 factors, which were peripheral influence purchase and conviction purchase. The processed convenience food selection attribute consisted of 4 factors, which were quality, convenience, packaging and price. Factor loading confirmation and reliability test were conducted, and the reliability was confirmed with Cronbach's alpha coefficients for all the factors exceeding 0.5. The high stress levels showed significantly high stress factors of housework, family relations and economic stress (P<0.001). The high stress group was shown to make purchases by recognizing peripheral influences (P<0.01). When the selection properties of processed convenience foods depending on different stress levels were examined, it was revealed that among the three groups, the low stress group least considered the price aspect (P<0.01). After deducting the factors, AMOS (Analysis of Moment Structure) was used to conduct the confirmatory factor analysis for verifying validity. The structural equation model was used to determine the path coefficient. From the processed convenience foods purchase attitude, the peripheral influence purchase had significantly positive (+) effects on convenience (P<0.05). Also, conviction purchase was shown to have significantly positive (+) effects on quality (P<0.05). Housework and family relation stress were shown to have negative (−) effects on processed convenience foods selection attribute, and economic stress was shown to have positive (+) effects, although no significant relationships were revealed.

ABSTRACT

PURPOSE: The consumption of processed foods has recently been increasing due to changes in the living environment. The purpose of this study was to identify the contribution of processed food to the nutrient intake of adult Koreans. METHODS: A total of 15,760 adult people in the 6(th) National Health and Nutrition Examination Survey (2013 ~ 2015) were included in this study. According to the Korea Food and Drug Administration's classification criteria for processed foods, the 24 hour dietary recall data of the subjects were classified as processed food or natural food. The processed food intake, nutrient intake and major processed food sources by food groups were analyzed. RESULTS: Men consumed more processed foods than did the women. Consumption of processed foods decreased with age, but it increased with the education level and the income level. The total daily processed food intake accounted for 68.1% of the total food intake. The food groups with high processed food intake were beverage, vegetables, cereals and grain products, fruits, and milk and dairy products in this order. The top food source of each food groups were beer, kimchi, bread, processed apple products, and milk. After adjusting for age, gender, and energy intake, all the nutrient intakes and percentage of dietary reference intakes for Koreans, except carbohydrates, were significantly higher in processed foods than in natural foods. The sodium intake from the processed food was 96.3% of total daily sodium intake. The intakes of nutrients from processed foods, excluding vitamins C, dietary fiber, iron, and vitamin A, were higher in men than in women. The intake of sodium from processed foods was highest for people of 30 ~ 49 years of age, and the intake of sodium from processed foods decreased for people over 50. CONCLUSION: Korean adults consumed more processed food than the natural food, consuming more calories and most of the nutrients from the processed food overall total daily intakes. The intake of processed foods is expected to further increase in the future, and nutritional education and research on the ingestion and selection of healthy processed foods are necessary.

ABSTRACT

OBJECTIVES: The purpose of this study was to examine the coverage of the current mandatory nutrition labeling system on the nutrient intake of Koreans. METHODS: KNHANES dietary intake data (2013) of 7,242 subjects were used in the analysis. KNHANES dietary intake data were collected by a 24-hour recall method by trained dietitians. For analysis, all food items consumed by the subjects were classified into two groups (foods with mandatory labeling and other foods). In the next step, all food items were reclassified into four groups according to the food type and nutrition labeling regulations: raw material food, processed food of raw material characteristics, processed foods without mandatory labeling, and processed foods with mandatory labeling. The intake of energy and five nutrients (carbohydrate, protein, fat, saturated fat, and sodium) of subjects from each food group were analyzed to determine the coverage of the mandatory nutrition labeling system among the total nutrient intake of Koreans. RESULTS: The average intake of foods with mandatory labeling were 384g/day, which was approximately one quarter of the total daily food intake (1,544 g/day). The proportion of energy and five nutrients intake from foods with mandatory labeling was 18.1%~47.4%. The average food intake from the 4 food groups were 745 g/day (48.3%) for the raw food materials, 54 g/day (3.5%) for the processed food of raw material characteristics, 391 g/day (25.3%) for the processed foods without mandatory labeling, and 354 g/day (22.9%) for the processed foods with mandatory labeling. CONCLUSIONS: Although nutrition labeling is a useful tool for providing nutritional information to consumers, the coverage of current mandatory nutrition labeling system on daily nutrient intake of the Korean population is not high. To encourage informed choices and improve healthy eating habits of the Korean population, the nutrition labeling system should be expanded to include more food items and foodservice menus.
